Also known as Memo Pad Holder - I belong to a wonderful Yahoo Group called Technique Junkies. A member posted to the group how to make this wonderful desk set. Then I made up a couple and posted what I did to the group. I got lots of questions on how to make these up. So, I wrote up instructions and put them in a PDF document. Example
